Guided Evaluations and Forms- when you need something specific other than what is offered

You can skip domains and questions that you don't need when performing a guided evaluation with your student/patient.  But what if you need a specific guided evaluation for your niche setting?   

O.T. Wizard has the most commonly used pediatric templates for guided evaluations, but we always want to hear from you if you don't find what you need.  If you have a niche pediatric setting where none of the available guided evaluations work for your needs, then reach out to us via email or scroll to the bottom of knowledge desk and click "create ticket" and add your needs.   In order for our team to consider creating a new template, we will need a copy of the eval template that you currently use and also a copy of a PHI free evaluation (using the same template).  We will review it and get back to you with any questions.  

This is the same for Forms/Questionnaires.  

The main consideration for O.T. Wizard is that the template can be used by many other OTP and not just specific to a particular OTP or OTP group.
