Functional Life Skills & Vocational Transition Guided Evaluation

Overview

This guided evaluation is intended for individuals ages 11–21 with intellectual, developmental, or related disabilities who require transition-focused evaluation beyond standard developmental formats. It measures performance across five scored domains (Personal ADLs, Instrumental ADLs, Fine Motor & Functional Tool Use, Vocational & Work Readiness, and Community Participation & Safety) using a 0–4 ability rubric that recognizes AT-supported and strategy-supported independence as valid outcomes at every level. Rater flexibility allows scoring from direct observation, caregiver, teacher, or student report, and results are designed to directly inform transition IEP goals and skilled OT service justification under IDEA transition mandates and insurance medical necessity criteria.

How it works

Open the Guided Evaluation called "Life Skills and Transition".   You'll need to complete at least 10-20% of questions in each domain in order to produce accurate results.  You may skip around questions so that you can answer many that the student is Independent in, as well as other skill areas that would be classified as "dependent" , or other levels of assistance.

Step-by-Step guide

1. Guided Evaluations
2. Select a client that is at least 11 years old
3. Answer at least 10-20% of questions in each domain
4. Add a few clinical observations at the end
5. Finish
6. Create your report: Go to Report Wizard <Choose Client<Choose Full Evaluation <Educational or Medical (Audience)< Generate


7.  Create your Plan of Care (with goals): Go to Plan of Care <Choose Client <Choose the completed report < Choose Educational or Medical (has to match the report type) <Generate



8.  After reviewing and editing your POC, be sure to "activate" it so that the new goals show up in your session notes.

Save the goals that you like to your goal bank and easily reuse them later.
Warning
Common Mistakes
Your Evaluation Audience (Educational vs. Medical ) has to match your POC Audience.  If you changed your mind about the audience you want, recreate the report (using the other audience type) and then you can create your new POC for that audience.
