Purpose:
O.T. Wizard forms make it easy to collect consistent, rater-based data from caregivers, teachers, or other professionals. Each form is scored automatically on the backend, and results can be merged into your final O.T. report. Using forms throughout the evaluation process helps you gather participation-based insights, improve efficiency, and produce more meaningful recommendations.

All forms are automatically scored using standardized formulas (Percent Mastery, Likert Normalization, etc.).
Results appear as performance bands (Mastered, Developing, Emerging, etc.).
When you generate a report, simply select the forms you’d like to merge into your final narrative.
Each form’s data contributes directly to the child’s participation profile for richer, multi-rater reporting.
Scenario: You are evaluating a 6-year-old with attention and sensory concerns.
Workflow:
Send the ENGAGE-S and Brief Sensory Questionnaire to both parent and teacher before the evaluation.
Review results to identify participation gaps.
Administer the Executive Functioning Questionnaire (Ages 4–7) during the session.
Merge all forms into the final report to support data-driven recommendations
